SUPPLEMENTAL HEALTH FORM

CORONAVIRUS (COVID-19) Update

We are excited to announce that our office is now able to schedule appointments, but at a reduced capacity for the time being. We are so excited to see you! It is our goal to ensure the safety of our patients and staff while providing optimal care.

As we enter the re-emergence phase of this crisis, I am writing to let you know of the changes in our workflow and infection control. Our goal is to exceed the standards suggested by the CDC and other governmental agencies to provide the safest environment for our patients, staff, and families.

Updated protocols include:

Heightened infection control including advanced disinfection of the rooms as well as entire office

“Fogging” of all areas with environmentally safe high-level disinfectants

Plexiglass barriers for the business office

Use of ADS Extraoral Dental Suction to minimize air contaminants produced during treatment

A reduction in touchpoints

Paperless communications including electronic submission of health forms

Universal precautions with N-95 masks, full face shields, disposable gowns and shoe covers

No contact with other patients in the practice

As we continue to plan for this “new normal” and prepare to move forward together in the best way possible, we want to keep you informed about some of the latest changes you will experience at our office.

Before Your Appointment

To ensure patients arriving are healthy, one day prior to your appointment you will receive a Supplemental Health Form which can be filled out electronically and submitted directly to our office. This form needs to be completed prior to your/your child’s appointment. We ask that anyone accompanying the patient remain in their car while the patient is seen.

During Your Appointment

As soon as you arrive at our office, please CALL US. You will be asked to wait outside in your car. We will call to have the patient come in as soon as we are ready for you. This will reduce the number of people in the office and decrease patient to patient contact. BE SURE TO WEAR A MASK TO YOUR APPOINTMENT. Upon entering the office the patient’s temperature will be taken with a non-contact infrared digital thermometer.

After Your Appointment

You will be escorted to the front desk very briefly, for any additional information/arrangements and to make any follow-up appointments.

Remember, regular dental visits at your restorative dentist are an essential part of your overall health. We will help you re-establish your normal preventive program with our colleagues.

Braces Patients and Broken Brackets

We are unable to fix broken brackets if we do not know about them ahead of time. If you do not give us advance notice of a broken bracket during the rescheduling process, you will need to come back to the office for an additional appointment. This is an infection control issue, not a convenience issue on our end, so we appreciate your understanding.

Patients with Retainers

Finally, for those patients wearing retainers, we will reach out to reschedule your appointment. Please know that we will be pushing retainer checks out 3–4 months in order to make room in the schedule for those undergoing active orthodontic treatment. That being said, if you are having a problem with your retainers (or you have lost them), please call the office ASAP.
